Polyneuropathy
Polyneuropathy represents a condition of extensive involvement
of peripheral nerves by various causes. About 3% of the population over 55 years will be affected by a polyneuropathy.
Great is the contribution of nerve conduction studies and electromyography (EMG) in the differential diagnosis
of polyneuropathies.
Symptoms of polyneuropathies:
-
Numbness and pain (usually "burning") of feet and legs, particularly at rest and at night.

Reduced muscle strength particularly of limbs and especially the legs.
-
Sweating disorders (increased or decreased sweating of soles and palms).
-
Restless legs.
-
Reduced or eliminated tendon reflexes.
Myopathies and Myasthenic syndromes
-
Absence of skin sensory disturbances.
-
Delayed relaxation of a contracted muscle (myotonia)Deterioration of the weakness during the day (myasthenia gravis).
-
Improving muscle strength immediately after exercise (myasthenic Lambert-Eaton syndrome).
-
Sometimes muscle pseudo-hypertrophy (legs).
-
Muscle pain (in myositis and metabolic myopathies).
Symptoms:

Muscle weakness.
-
Muscle atrophy, usually in the proximal parts of the limbs
Carpal Tunnel Syndrome (CTS)
CTS is a very common condition
(up to 2.7% of the population).
Confirmation of its diagnosis is done by
nerve conduction studies and sometimes by electromyography.
CTS symptoms:
-
Numbness, tingling and pain of fingers and hand, sometimes extended to the arm and shoulder, especially at night.

Worsening of symptoms with manual work.
-
Subjective hand swelling and stiffness.
-
Clumsiness when grasping objects, often dropping things off hands.
-
Thumb atrophy.
Headaches - Facial pain - Investigation and Treatment:

Migraine

Tension headaches

Cluster headaches

Trigeminal neuralgia

Headaches with autonomic involvement
Multiple Sclerosis (MS)
Clinical and Laboratory Investigation of patient with probable MS (visual, auditory and somatosensory evoked potentials).
MS patient monitoring and disease treatment.
Symptoms:

Memory disorder, originally concerning
recent events.

Attention difficulty.
-
Disordered orientation at a place and time (eg can be lost on familiar environment or does not know the present day, month and year).

Difficulty in simple arithmetic calculations.
-
Behavioral disorders, eg aggression or apathy.
-
Failure to recognize familiar persons.
Dementia - Alzheimer's Disease
In our neurophysiological laboratory we perform many specialized neurophysiological tests such as:

Nerve conduction studies as well as F wave and Hoffman reflex studies.

Electromyography.

Nuromuscular junction function studies (presynaptic Lambert-Eaton, postsynaptic Desmedt).

Blink reflex to investigate trigeminal and facial nerve, as well as brain stem, function.
-
Visual evoked potentials (VEP, potentials of the occipital lobe) to test the function of the optic nerve.
-
Brain stem auditory evoked potentials (BAEP) for auditory nerve and brain stem function testing.
-
Somatosensory evoked potentials (SEP) to investigate peripheral and central sensory pathways.
-
Entrapment neuropathies.
-
Traumatic peripheral nerve lesions.

Large and small fiber neuropathies.

Radiculopathies.

Disorders of neuromuscular junction.
